This module begins the process of weaving all of our new knowledge and skills into something usable and meaningful. It will demonstrate how to use the tools you are now familiar with to create a personal plan for each person living with dementia.
Once we know what approaches work, we can REPEAT them over and over.
Once we know what triggers a stress response we can tell everyone involved how to AVOID them.
A personalized care plan, that everyone follows, makes the difference in quality of life for people with dementia and their care partners. It is a roadmap to initiating a good mood with the person with dementia, maintaining it throughout the day, and saving energy for quality of life moments.
